A number of RHF communities have a Service Coordinator on site to assist residents in obtaining the necessary services they may need to maintain their independence.
What can a Service Coordinator do for you?
Service Coordinators can provide a valuable service for you. The following is a list of some of the things your Service Coordinator can do for you:
Financial Assistance

Assist you in reading and understanding your bills and place calls to companies when there is an error.

Sign you up for benefits with companies who offer discounted services on phone services and other necessities.

Refer you to banks, financial institutions and other agencies that can help you with balancing your checkbook or other personal finance issues.

Advocate for you when you may have problems paying bills or work with companies who have payment plan options.

Provide assistance with entitlement programs.
Medical Assistance

Provide assistance with prescription drug benefit plans and find companies that offer low-cost prescription medications.

Assist in setting up a home health aide or needed nursing services.

Set up "wellness clinics" where healthcare providers come to your community and offer free blood pressure screenings, cholesterol tests, and other health tests.

Help you read and understand your Medicare and Medi-Cal bills or statements.

Arrange physician appointments and transportation services.

If you are admitted to the hospital, they can work with discharge planners to make sure your transition back home runs smoothly.

Arrange for home medical equipment delivery and set-up.

Distribute and help you fill out Confidential Emergency Information forms for paramedics to use in case of emergency.
Homemaker Assistance

Assist you in finding and arranging for homemaker assistance (cleaning, cooking, laundry, etc.).

Refer you to grocery shopping services.

Arrange for meals-on-wheels or other food preparation services.

Arrange transportation services for shopping and other needs.
Educational and Emotional Support

Provide monthly presentations on topics related to: living wills, telemarketing fraud, home health providers, computer training, companion programs, etc.

Connect you with other educational and recreational programs in your community.

Work with libraries and MOWs to have books and other resources delivered on a routine basis.

Set up senior companion programs or "buddy" programs for safety or crime watch services.

Arrange counseling services for loss, grief, etc.

Connect you with on-going church services, bible study programs, etc.

May provide mediation assistance.
Legal Issues

Assist you in completing forms or obtaining necessary information regarding living wills, advanced directives, and other forms.

Assist you in reviewing your social security earnings statements.

Assist you in reviewing death or survivor's benefits, or making funeral arrangements for a loved one.
Service Coordinators will work for you in your community to find agencies and support programs to assist you and the other residents of your building.
What a Service Coordniator may not do:

Service Coordinators are not permitted to provide direct services, which mean they may not perform the duties of a home health aide or a homemaker, but they will find the services you need and arrange them for you.

Service Coordinators are not permitted to provide direct financial assistance, but they will find the service and arrange it for you.

They are not permitted to provide or recommend medical information, but they will find the service and arrange if for you.
